GOALS:

• Strengthen Community Partnerships
• Break down barriers for HIV Prevention through ongoing coalition-based meetings
• Create stronger linkage to care for HIV+ community members
• Closing the loop on referrals & checking in on status of referrals
• Discussing referral grievances

OBJECTIVES:

• Identify specific barriers in HIV Prevention (PrEP especially) amongst priority populations
• Be able to understand where community members fall out of HIV Preventive care and create system/workflow to maximize stakeholder resources
• Message & outreach HIV Prevention to the Sacramento Region in a more tangible way by understanding community members lived experiences & stories
