Quality: Nothing is more important than the quality of work we do with our patients and in every department that supports that care. We have a Quality Plan and look to constantly improve in every aspect of our business.
Academic Partnerships: We collaborate with our local hospitals and with the University of California-Davis to provide opportunities for fellowships and residency.
Research: We have been involved in research studies with UC Davis for several decades, allowing our patients to have access to new treatments.
Team Based Work Space: Our newly remodeled clinic has space for teams to work together to enhance collaboration.
Clinical Pharmacy: Our on-site Pharmacy is supplemented with a team of Clinical Pharmacists who consult with patients, help manage pain contracts, and assist with Pre-Exposure Prophylaxis (PrEP).
Integrated Behavioral Health: Our Licensed Clinical Social Workers are part of the team and can assist patients in compliance with their treatment plan as well as link them to other resources that may be barriers to wellness.
Supportive Services: We have many resources in-house to help patients including mental health services, substance abuse treatment, nutrition and diabetes educators, case management, linkage to community resources such as housing, and much more.
Epic Electronic Health Record: We use Epic as our EHR through a partnership with OCHIN.
Growth: We have the staff and the resources to grow. We are constantly looking for opportunities where our skills match the need in the community.
Advocacy: We are an Advocacy Center of Excellence and actively let our views on healthcare be known on the local, state and national level. Because of our work, we were given Gold Status by the National Association of Community Health Centers, one of only 21 in the country.
Community Events: We love to get out in the community and talk about what we do. We go to health fairs, sponsor events, and participate in walks for various causes. We can be found at the annual Pride festival, MLK 365, and at Celebrando Nuestra Salud.
Food Giveways: We know that access to healthy food can be a challenge for some people so we have a fresh produce giveaway every month. And we make it easy to know where to get food at a network of food banks and farmers markets for the rest of the month. In December, we give away everything needed for a traditional holiday dinner.
Fun Events: Twice a year we have fun staff events. Sometimes its a parking lot picnic and other times a holiday party. We mix it up and have a great time.
Mission Driven: Last, but not least, we are mission driven. We are here to transform lives and make our community stronger.
