
Important Notice: One Community Health is notifying patients about a data security incident involving TriZetto Provider Solutions, a subcontractor of our partner vendor OCHIN. This incident did not involve One Community Health’s systems. We take the privacy and security of your information seriously.

What Happened
On October 2, 2025, TriZetto Provider Solutions (a subcontractor used by our partner vendor OCHIN for insurance clearinghouse services) discovered suspicious activity in their web portal. An investigation determined that between November 2024 and October 2, 2025, an unauthorized individual accessed certain historical eligibility transaction reports in TriZetto’s system.
What Information Was Involved
The information varied by individual but may have included: name, address, date of birth, Social Security number, health insurance member number, health insurance company name, primary insured/dependent information, and other demographic and health insurance information.
The incident did NOT involve: Payment card information, bank account information, or other financial account information.
What We Are Doing
TriZetto’s Response:
TriZetto engaged cybersecurity experts (Mandiant) and law enforcement, secured their systems, and confirmed no unauthorized activity since October 2, 2025.
One Community Health’s Response: Although this incident did not involve OCH’s systems, we immediately conducted comprehensive security checks of our infrastructure—no breaches were found. We notified all potentially affected patients and reported the incident to state and federal agencies (California Attorney General and HHS OCR) within required timeframes. We are coordinating with OCHIN and TriZetto to ensure affected individuals receive information about complimentary credit monitoring services.

Free Credit Monitoring Services: TriZetto is offering complimentary credit monitoring, fraud consultation, and identity theft restoration services through Kroll at no cost. Affected individuals will receive separate enrollment instructions from TriZetto.
What You Can Do
We recommend that you remain vigilant by reviewing account statements, monitoring free credit reports for suspicious activity, and reviewing Explanation of Benefits statements for services you didn’t receive.
