Video Telehealth Visits (Epic MyChart)

One Community Health is using secure video-based telehealth visits to ensure quality care, improve patient experience, and meet new California health plan requirements.

What Video Telehealth Visits Mean for You:

  • Telehealth visits are completed using video (MyChart + Zoom).
  • Phone-only visits are available if you are unable to use video technology.

What You Need:

  • A smartphone, tablet, or computer with camera and internet.
  • A MyChart account and login (ask us if you need help setting it up).
  • The free Zoom app (no account or payment needed).

Step-by-Step Guide to Video Telehealth Visits:

Step 1: Download MyChart.

  • First, use your smartphone or tablet for the visit (recommended).
  • Next, open the App Store or Google Play and search for ‘MyChart’.
  • Then, download and install the app.
  • Finally, when prompted, select OCHIN as your organization.

Step 2: Log in and Test Access.

  • Open the MyChart app and log in using your credentials.
  • Make sure you can access your account without any issues.

Step 3: Complete PreCheck-In (Day of Visit).

  • Log in to MyChart 15 minutes before your appointment.
  • On the website: go to Visits > Appointments and Visits.
  • On the app: go to Appointments.
  • Select PreCheck-In and:
    • Confirm or update your personal information
    • Review medications, allergies, and health issues
    • Sign any required consent forms

Step 4: Start Your Video Visit.

  • After PreCheck-In, tap Begin Video Visit.
  • Zoom will launch in your browser with the message: ‘Please wait for the host to start this meeting.‘
  • You don’t need to click anything else—your provider will start the session.

Step 5: Reconnect if Disconnected.

  • If disconnected, return to MyChart and tap the green Begin Visit button again.
