
One Community Health OneHeart Program Kickoff
The OneHeart program is part of One Community Health’s Kaiser PHASE (Preventing Heart Attacks and Strokes Everyday) grant. RNs are able to support patients through frequent visits, patient education, goal setting, lifestyle modification and medication valuation. Patients in the OneHeart program are further supported through referrals to One Community Health’s Clinical Pharmacy, Nutrition, Behavioral Health and Case Management services as needed.
Uncontrolled high blood pressure increases the risk of serious health problems, including heart attack and stroke. One Community Health’s OneHeart program provides our patients the support they need to lower their blood pressure and improve their health.
