GOALS:
- Strengthen Community Partnerships
- Break down barriers for HIV Prevention through ongoing coalition-based meetings
- Create stronger linkage to care for HIV+ community members
- Closing the loop on referrals & checking in on status of referrals
- Discussing referral grievances
OBJECTIVES:
- Identify specific barriers in HIV Prevention (PrEP especially) amongst priority populations
- Be able to understand where community members fall out of HIV Preventive care and create system/workflow to maximize stakeholder resources
- Message & outreach HIV Prevention to the Sacramento Region in a more tangible way by understanding community members lived experiences & stories
