One Community Health has a long history of providing behavioral health and substance abuse services to primary care patients. As a Patient Centered Medical Home, we care for the whole person—body and mind.
We use a collaborative Integrated Behavioral Health model. This model provides behavioral health interventions as part of the care delivered to the patient. Having behavioral health staff available in the clinic setting allows patients the opportunity to obtain needed behavioral health services through a “warm handoff” from their primary care provider, in a more streamlined and direct manner.
