Street Medicine
One Community Health believes everyone deserves access to quality healthcare — no matter where they live. Our Street Medicine program brings essential medical services directly to people experiencing homelessness or living in unstable housing. By meeting individuals where they are — on the streets, in parks, or in shelters — we break down barriers to care and offer support with compassion and dignity. If you or someone you know needs help, contact us to learn more about how we can assist.

Street Medicine Services in the Sacramento Area
The Street Medicine program at One Community Health brings healthcare directly to individuals living outdoors or in unstable housing. Our team offers primary care services in the field, including routine checkups, lab collection, point-of-care testing, and some medications. We’re also working to expand services to include dental care, behavioral health, medication-assisted treatment (MAT), and care coordination.
Here’s a look at what we offer (and what’s coming soon):
Primary Care Where It’s Needed Most
Our Street Medicine program starts with primary care, delivered directly in the field. Whether someone needs a basic checkup, help managing a chronic condition, or follow-up care, we bring services to unsheltered individuals where they live. One Community Health’s goal is to reduce harm, build trust, and meet urgent health needs head-on.
Here are some of the services we currently offer during outreach:
As the program grows, we are also working to include:
Through this model, we bring patient-centered care directly to people who need it most.
Our Commitment to Street Medicine
At One Community Health, we believe healthcare is a human right. Our Street Medicine program was created to bring care to those who are often left out of traditional healthcare systems. We know that individuals experiencing homelessness face unique challenges, and we are here to meet them with compassion, dignity, and respect.
Our goal is to remove the barriers that keep people from getting the help they need. That’s why we go directly to the streets, parks, and encampments — because no one should have to choose between survival and their health.
As our Street Medicine program continues to grow, we remain focused on building a healthier, more connected community by showing up, listening, and delivering care where it’s needed most.
When Should Someone Receive Street Medicine Care?
Minor Symptoms That Shouldn’t Be Ignored
Even if the symptoms seem small, our team can help treat the issue early before it becomes more serious. We also offer health checks for people managing chronic conditions like diabetes, asthma, or high blood pressure.
When Emergency Help Is Needed
Street Medicine is not a substitute for emergency medical care. If you are experiencing severe or life-threatening symptoms, please call 911 immediately. Our services can connect individuals with emergency care only during operational hours. After hours, patients will be directed to the OCH after-hours care line for triage.
When we’re out in the community, you can talk with us directly. Or, if you’re supporting someone in need, contact us for guidance — we’ll help you figure out the best next steps.
