Supporting One Community Health Diabetic Patients

One Community Health is committed to improving the lives of our diabetic patients and helping them to better manage their diabetes. During specially scheduled Diabetes Focused Visits, patients will receive comprehensive diabetes care based on the American Diabetes Association 2019 Standards of Care. These visits will include necessary labs, referrals to specialists and medication adjustments.

We want to make sure patients have the tools and support to learn more about diabetes, and with their provider’s help, take steps to lead a healthier life. One Community Health providers will be working directly with their diabetic patients to schedule these focused visits.
